SUBSTANCE ABUSE ASSESSMENTS COSTS
Substance Abuse Assessment for Court or Probation – $250
Secretary of State Substance Abuse Assessments (SOS-258) – $450
Substance Abuse Assessment to return to Work (Non-DOT) – $250
Substance Abuse Assessment for National Guard or Military Service – $250
PAYMENT
Unfortunately, no – assessments for legal issues are generally an out-of-pocket expense.
You will be required to enter in your credit or debit card when you schedule your appointment online. When we accept your request, we will charge your credit or debit card on file at that time to reserve your assessment appointment.
If you cancel your appointment at least 48 hours before your scheduled assessment, you will receive a full refund. If you cancel your appointment within the 48 hours before your scheduled assessment or no-show for your appointment you will receive a 50% refund.
If the substance abuse specialist that you are scheduled to see is not available due to sickness, you can either reschedule or receive a full refund.
SUBSTANCE ABUSE ASSESSMENT
You should allow 90 minutes, but generally interviews are completed within 60 minutes.
Your counselor will call & text you.
We use a secure HIPPA compliant platform (Simple Practice). After scheduling your assessment, you will receive an email with all the required releases of information, HIPPA forms, and testing required in one email to fill out online before your scheduled interview with your counselor.
You can use any computer/laptop/tablet that has a camera or your cell phone.
CONFIDENTIALITY & COMPLETED ASSESSMENT
All assessments are confidential. You and the referral source (Attorney or Probation Officer) that you select are the only people that receive your assessment. We will email your assessment in PDF format to you and your referral source.
