  • You must enter in your credit/debit card to reserve & pay for your assessment.
  • You will receive an email prompting you to fill out the assessment paperwork in your portal.
  • You will receive appointment reminders via email & text message 48 & 24 hours before your scheduled appointment.
  • If you need to reschedule or cancel your appointment, you must do so within 24 hours of your scheduled appointment to receive a full refund.
